Description

We need to have an Adobe interactive form linked and automatically loaded into a MS-Access database. Currently the form is sent and received via email. We would also like to have the form or similar on a website and also linked and automatically loaded to the same MS-Access database.

There are approximately 15 forms to be linked/integrated to the database.
