Hi,

I am looking for a PHP 'EXPERT' to build a php booking form for a website, the form needs to accept input from the user and when submitted it will need to send an email and send the user to a paypal payment page. The form will also need to calculate the different variables in order to ensure the user pays the correct amount.

Below is the correct order for the form fields and instructions that are needed to process the order:

* Name
* Contact email
* Phone
* Service
o Hourly booking £20 per hour
o Half day session £50
o Full day session £85
* Select a date
o Start date [Add a calender here, the user will need to pick a start date, user cannot select a date that has passed]
o Start time [Option box so the user can select start time, if they have selected 'Half day' the options are morning or Afternoon, if they have selected 'Full day' the options are disabled, if they have selected hourly the options are 8AM, 9AM 10AM, 11AM, 12AM, 1PM,2PM, 3PM, 4PM]
o End date [Add a calender here, the user will need to pick a end date, user cannot select a date that has passed ]
o End time [Only for Hourly Booking' options are 9AM 10AM, 11AM, 12AM, 1PM,2PM, 3PM, 4PM] The system needs to calculate how many hours are booked add to the total.
* Course name
o Editing
o Graphic design
o Web Design
o Internet access
* Do you require editing assistance (£25 per hour)
o 1hour [£25 will be added to the total]
o 2 hour [£50 will be added to the total]
* Number of machines required [For each additional machine the system will add the service cost e.g. if it is a booking for hourly and they have 3 machines this field will multiply the service cost 3x20=60 , e.g.2 for 5 machines at 'Full days session' the cost will multiply by 5 which will be 5x85=425

* Total [This field will show the total that the user is required to pay]
* Paypal buy it now [Button that will take the user to the paypal site where they will pay. On clicking the submit button an email with the buyers details and the order will be sent to the admin email account]
