Description

i need a form that capture name, address, zip, etc. this form will have a button called ready to order. Once this button is clicked, the customer will go to a one page check out form where the name, address, zip, etc will be pre-filled from before. This one page checkout will also have a credit card, cvv, and date of expiration field that will be entered by the customer. This form will be on a secure server, and the credit card must be validated with expiration date, etc.

I will need to be able to add my merchant account and processing company together with the price of the product. its only one product and cost 99.99 USD.

at the end all the information entered will be stored on a mysql database

new update for the project. i just need the one check out page. will us authorize.net and need to be able to add the merchant account and pin etc. this form will have credit card info, echeck if they decide to pay with echeck and paypal. once they click submit order the order will be placed. i need this in 2 weeks the most.
