Description

Title says it all. Two page pdf, designed for user to filll in. Would like to make it fillable form with data entry areas and check boxes etc. Also, several date calculation. ie user enters date in one area and it autocalculculates corresponding date in other area. Also, if one value is marked as yes or 'checked' another area will be highlighted to draw attention to it.
