SUMMARY:

We need to build a web based interactive application for a receptionist. She would answer the phone and take a message for a particular extension number that she will enter into the extension field on the web form. Then she will take the message by populating the rest of the fields which will be returned automatically based on caller’s answers to receptionist’s call guiding script. In other words, the Receptionist’s Form will be built differently for each incoming call based on the preceding answers of the caller.

Once she clicks on the Submit button, that extension number would have to find itself in MySQL database along with a corresponding email addresses in the same row. Once the match is found, then the content of the form should be emailed to those particular email addresses.

Each extension number will have a unique email address specified in the database which would have to be administered through a web login page. Likewise, the page that our receptionist will use should have a login page as well.

This application must be compatible with both hosting operating systems – Windows and Linux. Also, it should be compatible with most widely used internet browsers and support simultaneous transactions since there going to be more then one person answering calls at a time.

Additional features are necessary to advance this application and you will find most likely all of them explained in detail in the attached MS Word file. Please give me your very best bid price (keeping in mind the BONUS that I mention at the end of this page) along with the answers to this set of questions below in order to be seriously considered as we are looking forward to have this project started and completed as soon as possible, yet without any short cuts that will cut into the efficiency and performance of this application.
