Description

I need to create a website with 3 modes.

User
Administrator
Control Panel

User – Where multiple users can login with his own login ID and password and enter his details (basically entering details in to a survey form). He needs to have an option to view the submitted results with respect to his ID.

Administrator – Where the details can be edited later and either of there status could be provided. (Approved, Pending, Rejected) along with a text box to enter comments.

Control Panel – Where you can add/modify/delete data submission fields. This means in case a data submitting field is a ‘text field’ it can be changed to a ‘drop down menu’ or ‘date window’ or ‘comment box’ or a ‘check box’ kind of field.

The database is hosted on a secured place and I could access the database from my laptop.
