Description

Hello.
I need 2 simple forms to be created fast...i don't have time for them.
Please see the attached file for the form details.
Payment only in Escrow and no more than 40$ for this job...it takes around 1 hour for a good developer. I will not pay anything without a quick demo of the forms.

PS: the last provider said 2-3 hours to complete it and after that 9 hours!!!.
When you pm me that you can do it make sure you have the time for it.
deadline: max 6 hours.
Prefer Europe providers due to the timezone.
