You could simply hook the CreateFile or CreateProcess called by every application for opening or creating a new document. Using this method, you will be able to capture the application name and what document they opened.

Hello I had a look for primary design and I doubt it could be determined the program that opened the file.However it could be found the changed file.Just register for action for directory and if it happens(OS signals it) the directory could be scanned and file parameters compared with the old(Database saved in memory) and save changed files in text file.
