Description

I need a custom Outlook (2003) form that:

1. Is easy to deploy to multiple workstations (we use Exchange 2003 so a public form might work)

2. Launches from a toolbar button in Outlook

3. Takes input from multiple fields on the form and places all of the field input into the subject line of an email. Only one

field will go into the body, the rest will go in the subject line.

4. Does NOT trigger any security dialogs or require any complex workarounds or kluges.

5. Can be modified by me after it's been developed.


The form is simple, it's just a While You Were Out form. It needs these fields:


* Date & time (auto-filled with current time & date but user-editable)

* TO: (auto-filled with default value but user-editable with a TO: button that opens Outlook address book)

* Caller Name:

* Phone Number:

* Wants return call: (checkbox)

* Urgent: (checkbox)

* Notes: (large text box)


Except the Notes field, ALL of fields will go into the Subject line, preceded by the string "PHONE MESSAGE: "

Like this:

SUBJECT: PHONE MESSAGE: John Smith 555-555-5555 Please Return Call Urgent

-- or without the checkboxes checked --

SUBJECT: PHONE MESSAGE: John Smith 555-555-5555

All this info would be repeated in the body of the email, and anything entered into the Notes field would also go into the body

of the email:

to your meeting today. Please give him a call if this is
