I have a need to have 4 forms programmed for use separately or any combination for numerous sites on same server.

Program will:

Each form is to have single black border with background color of Pale Yellow (#FFFFCC).

Forms are to be in a table

1. Create data fields in HTML for each form and assign them to My SQL

2. Required entries have asterick before the field name.

3. As data is input by site visitor, provide error coding for required entries, and be sure email and phone numbers entered incorrectly. Do not accept phone 555-1212 - reject with error code
No field is to start with a special character

4. When Data is entered, it is stored in database by date and time for that form and and copy of the input is emailed to (my email address I will provide) showing both the field name and the data entered ---

such as.......

etc etc etc

5. Send an autoresponder message to site visitor who input data....Make it so I can change message (250 charcaters)

NO DATABASE IS NEEDED. Just program it to accept input in each field and then send me an email with the field name or question along with the users input ....

That will be perfect, and of course send them an autoresponder message that I can change as I wish...

Let me run a test of each form then you can send the files to me via zip.file or FTP them if you wish....

Well determine that after I see the finished product and run a test on each one first.
