Description

When someone decides to exit my site, I want them to see a form that asks them to sign up for our email list. We want to use constant contact. Right now we have vertical response custom integrated into a field on our website. We want it where people enter their email address in the field and hit submit and they don't have to do anything more if possible.

So what I am looking for is the form and the code. We want the form to match the feel of our site somewhat, or at least have our logo on it. It would also be ideal if it would be easy to turn the form "on" or "off." You can just send the code/files to me with instructions on how/where to put the files, and I will add them to our web server. $30 budget.
