Description

We need great graphics that are related to mobile service and mostly Blackberries,the website should be mostly white with some red and black colors. Under our logo should be the following tabs: home page, sign up now, about us, contact us, support. To get a feeling on what we are looking for as far as graphics and the general look of the website please visit sprintpcspromotions.com

On the bottom of the web pages should be a red line, under the line should be the following links: home page, sign up now, about us, terms and conditions, contact us, support.

Our home page needs to advertise our rate plan, under that should be a sign up now link that points to the sign up page and a login and password form in the upper right corner.

The sign up now page needs to have a form with the following fields:

On the top of the page it should state the following:
Please provide your contact information on what name you want your new Wireless Promotions account to be opened under. (not your info on file with your previous mobile phone provider)

The following fields to fill in
customer name*
contact phone number* on the right side should say not your phone number that you are porting
Email address*


There should be a "check box" for customers who are not porting an existing phone number from another carrier and want to get a new mobile number. (if this box is checked off then the following 4 fields should become grayed out and not be required to fill out)

add a field box "called "Notes" where customers can write a detailed note



a submit button on bottom, if any fields are left blank they should not be able to continue without filling in the required fields.
Once they click on “submit” an email with all the info should be sent to an email address
and say Thank You!

there should be a forgot password link where they can submit there user name and get an email with their password we should be able to reset their password from our portal as well

when they login they should see the following info all entered by us when we create their login name

all should be able to be changed at any time by us

This project has to be done in 3 days of winning the bid
