Description

I am looking for a simple website design for people to be able to upload their pet information onto a free website so if they are travelling and their pet needs to see a vet they can simply go to this website and download their pets information. Three examples of layout are as follows:

http://epetrecords.com/demorecords.asp or
http://www.petevents.com.au/petrecord/index.php3 plus upload vet records
http://www.yourpetrecords.com/ (I like this one the best)

It needs to be very easy to use. Please see attachment for information about the layout and feel. This shouldn't take to long to finish and the design should be fairly basic. Please contact me if you have any questions. We can expand on the website as we go and add more to it later also.
