Description

In short I need this application form turned into an online wizard with a few questions per page,simular to the format of a paypal survey but employment related questions.

I need this ready for Tuesday 2nd Sept and will include a 20% bonus if this can be delivered on time.

I do not need any error checking the candidate can skip any questions except for their name, contact number and email. I need an option for them to submit their resume/cv if they have one. They can also opt to only submit a CV/Resume if they do not wish to also fill out hte application form.

I need the results saved to SQL and emailed to us and the candidate.

The questions that will need duplicating are attached in the form. I would also liek the form converted to PDF and as a downloadable option rather than completing it online.

This is to be hosted at www.phonetek-nation.co.uk/jobs the first page prior to the wizard will list available jobs, on this occasion I would like this to read "Sales Assistant" and Another link stating "Administrator" and these links would follow on to your wizard. One of the first questions will have to read from a database our list of stores (they can apply for multiple) and allow them to choose which store(s) they want to apply to work in.

For the right candidate this will lead to further work.
