Description
We are looking for a simple little website with an initial login page where you enter an email address and password (password twice during first time use). Then there is a form, divided over 5 or 6 subpages (previous, next buttons as well as "page 1 through 6" buttons). This form contains multiple choice, sliding scale and open questions.After the form is complete it should perform a calculation based on some of the questions (simple addition of points attributed to certain answers). After all this is complete, it should prepare a pdf that is emailed to our company address and the user email address.
If the user comes back, he can get to his information by simply entering his email and password. We would like to have a separate (secret) login page that allows us to access the information with just the user email (and possibly our admin password).
We would like this live asap and want to collaborate during central european working hours (amsterdam time, 9:00 to 18:00). Contact will be primarily e-mail and possibly msn. Design should look professional and attractive, but no need for 3d design or flash or such things.
