Description:	The uploaded form is a legal form. It serves a master form. I want a simple website built which allows a user to fill in various fields online and have the answers imported to a master form. Once the fields have been filled and "custom document" has been prepared from the master form I want the user to be able to preview the document and be able to confirm that it is correct. Once this is confirmed the file containing the "custom form" and instructions for use is ready to be downloaded. For this purpose a paypal download script needs to be integrated into the website to allow for delivery of the form. I am no programmer and I only know what I want so please feel free to suggest. I do note for example: http://www.hotscripts.com/Detailed/58029.html I am an attorney and if persons are shy to complete this online and want us to do this for them then we need a simple contact us form. We would use the same site to create the document and then print and mail the form to the client. There are some special situations not relevant here but suffice it to say that we need a couple of inquiry forms to accomodate these special situations. This too is very easy.
