Description

I have a form and I have trouble making it look like i want.
I attached the html file and a screenshot. It is a screenshot like the form should look like (screenshot made from the "design view" in dreamweaver). So everything must be aligned and sized exactly like in the screenshot.

This should be a very easy task and will not take more than 10 mins to do it. So i will not pay for this project.
But I will give a 10 review to the one who gives me the best results in the less time.
