I can design the customer survey script and database you desire. The script will have three pages, the first is for a user to enter a code which is checked against the codes in the database. The second will show the user the questions you provide. Once submitted, the results will be stored in the database, and emailed to you, and a third page will display your thank you message. The database will have a table for codes which will be marked used if appropriate so the code is not used again, and the email reminder will not be sent after the code is used. This table will also have the name and email information used to send the reminder email, and the date a reminder was last sent. The database will also contain a table with a field for the answers to your questions, the code used, and the time and date the survey was taken. I can add more information to this table if you like. There would also be a file provided by you which will be merged to send the reminder email. The script will run daily and send any reminders which are to be sent (you can choose 1 time or every # days.) If you would like, I can also create an administrator section where an administrator can login and list/sort/search the results, change the questions, change the reminder email, and change the thank you page (cost extra). I do request you send me the text content to be used on all the pages and in the email so it is just as you want it. I would like to build my reputation on here. I just subscribed here yesterday, but I have been working on computers and programming web sites and software for my clients since 1997. I don't let my client's down!
