Description

I need one psd file converted for an art-project website. It is very simple--just one page with an intake form. I want users to be able to fill out the following forms: name, mailing address, and a box for additional information.

Please let me know the following information: how long it will take, what country you are operating in, and the bid. I also would like to know how I will receive the information from the website intake form. I am new to this, and I would prefer to work with someone who is willing to explain things to me in decent English. Thanks for your help.
