Description: I want to build a one page sales letter website. To get an idea of what I am looking for please visit www.getfitwhileyousit.com

The main differences to this site are:
1. I have several "video testimonials" that I would like to be able to click on which stretch down the right side of the site.
2. There needs to be an opt-in box at the top of the screen where they can sign up for a "free newsletter". An automatic email should be sent to them thanking them for visiting our site. Then the email address should be sent to me for me to follow-up with in the office.
3. There should be a place where people can click on to get our "intenet special". This take you to a page which is a "certificate" that people can print out and bring to our office.
4. There should be another opt-in section where patients can "click here to receive a free toothbrush" Clicking that link should take them to an opt-in page where they can enter their name, addres, etc. This info then needs to be emailed to me.

The layout of the site should be the same as www.getfitwhileyousit. Same color scheme, a very long salesletter with a main headline and many subheadlines, many written testimonials, and many places where they can opt-in to give us more info.

I own the domain name and have hosting capabilities.

I have a series of keyword combinations that I would like the site to be optimized for.

We are a small company with a modest budget. However, this is jsut the first of many projects I will have. I like to remain loyal to providors who do exceptional work.
