Description

I need a credentialed web designer to redesign an existing website. All the contents will be copied from the existing site and should be added to the new site. This a medical site and we need the following:
1- Great looking clean design - you need to be patient and make changes to get the final results.
2- We need to have Google maps in contact us page to show dynamic map and directions
3- We need an appointment request page with a form that will be emailed to someone. Upon filling out this form, a confirmation email should go out to the person who entered the informaiton in the form and also one copy with the information is sent to another email address.
4-You can use flash header to make the site nice.
5- This site will be hosted on Godaddy linux server and you have to upload the site and make it work.
6- I will pay 30 dollar bonus for fast delivery and top notch work.
7- you should send me few mocks before starting the work.
8- Ask any questions before you bid.
