What you will be required to do:

In a given period of time, you will convert the site over to a new Joomla site which will be deployed on the same domain. I plan to move the sunshinespine (static) to a temporary location from where you will CREATE new content within Joomla.

I am going to deploy Joomla and i am going to upload a new template. No cosmetic changes are needed for the template.

You will setup categories / sections which will provide best SEO friendly URLs. Base level structure of the site listed below should stay the same:

index
conditions treated --> list of conditions + links to each of the conditions
injections performed --> list of injections + links to each of the injections
etc.
etc.
etc.

Please see the website to get the remainder of the structure.

Categories / sections and how the URLs are being structure must be approved by me before you go ahead and complete the work. I dont want surprises on this since this is the key requirement of the project.

NOTE: You will not deploy any plug-ins or anything else in Joomla. This should work fine using the base level Joomla 1.5 out of the box. Please point out in your bid if there is anything special that you see that needs to be accommodated.

Additional requirement that is going to be needed is a contact form where people can fill in a few fields (name, email address, phone, appointment time needed) and email is sent out containing that information. This is not mandatory but nice to have. Let me know how you plan to accomplish this while having this be available on all pages as a side widget possibly? Not sure since i am not the Joomla expert.

Last but not least, you will not simply copy and paste stuff from bad HTML (existing site) into the new Joomla template. Images must be moved into joomla and referenced locally. There should not be any <FONT> tags brought over into Joomla as well. Basically, you will bring over content / images without the bad HTML stuff.
