Description

Implement one web page that captures simple data from users into a .csv file. I will provide the content and layout, and an example of a similar page we did a few months ago. Part of the page will use radio buttons to limit selection to one item within each segment of the grid.

You can see from the attached .html the page we used earlier, and this one will be similar. Radio button selection is one per time slot.

Content is about the 200% of the amount in the attached example. Will provide this in an Excel format corresponding to the two column layout you see in the attachment.
