Description: I would like to add a web page for users to fill a form, click a button to select a file on their computer to FTP me a file. I also would like to record entered info to the form and send also. Perhaps zip the selected file (typically a *.mdb file ranging from 15 - 80 mb). Alsp maybe send me an email that a form also has been uploaded.
