Description

Looking to get a page in my site where a user can enter his email address and unsubscribe. Once email address is entered, a message letting the user know that the request has been processes should be viewable to the user. And the emails are saved on in a file that I can retrieve at the end of the week.

prefer the pages in html since we are using a template for headers and footers.
