Description: This is a telephone survey, pen and paper, for Philippines (130 interviews) . Lenght of interview is 10-12mins. The targets are pharmacies (80 interviews) and hospitals (50 interviews). The subject is 'self monitoring blood glucose'.

The budget is $6-$8 per interview for 130 surveys. The winning bid will be invited to do 3 more surveys for each quarter. In total are 4 similar surveys.

Quality Assurance requires voice recording for the interview. You can use Skype with voice recorder (Power Gramo, recommended). On completion, you need to submit the report in excel format.

Payment term is $200 on completion by escrow, balance payment is 30 days. Please do not bid if you are not agreeable to this term.
