We are looking to compile a database of doctors in the United States,

Fields required: (per list)

· Medical field -Specialty(required)
· Title (required) i.e. Dr., Mr.
· First name (required)
· Last name (required)
· Email (Prefer not required)
· Business name (preferred, not required)
· Full address (street, city, state, zip) (required)
· Phone (preferred, not required)

The list should be in an csv format and

The list must be relevant and information no more than 3 months old.

We have a variety of US nursing and medical practitioner jobs to follow.

If you have the data currently, I'll buy it.
If you know where to get it, give me a quote on scraping it.
If you want the job but need me to point you to sites then please advise me your cost.
