Need EMR and Its portal.there is open source code available or have a look at that praxis site , you can download their EMR product and I assume clone it

Must contains following features.

Medical Records

Medical History – Patient is able to fill this form on the website if they are a new patient after scheduling their appointment

Physical form – vital statistics, blood pressure count, heart rate etc

Superbill Form – cpt codes which physicians fill out regarding patients condition, diagnostic codes.

Patient Charts – with the ability to write or attach files with the chart (being able to store + reference corresponding files , lab results etc.

(we are looking to acquire these forms as examples = charts, superbills , lab forms etc)

HCFA Forms

Patient Hospital records

Drug & Pharmacy records

Physical Therapy records



Consultation Portal

Secure Patient signs in one of two ways

1.Physician name or assigned log in
2.patient log in

Templates : 1 gives the patient the ability to write notes to their physician
3.physician response page
4.online chat ability
5.ability to attach files , picture , text etc
6.payment method – credit cards , paypal or insurance

Appointment Scheduler

24hr scheduler

Divided into 4-15 minutes appointment blocks per hour
With notes capability , this is based upon the available time which the physician will set the parameters

Medical Providers Portal

Same as the consultation portal but geared for provider communications and transfer of documents , such as charts , medical history , procedures , lab results etc
Ex of providers , physicians , hospitals, clinics, labs , pharmacies and durable medical equipment etc
