Description: Client needs a CGI form solution written in Perl.

Script takes output from a one-page HTML form so that every time 'Submit' button is clicked, a new record is written in a Microsoft SQL Server 2000 table.

The data on the form needs validation with user prompt if validation fails. Once a successful submission occurs the user is taken to a "Thank you" page with a link.

3 of the fields need to be dynamic. The user selects from a list for field #1, which changes the values of field #2, which changes the values of field #3.

Solution to be deployed on onLOCAL ntranet and unavailable on the WWW.
