Description

I'm looking preferably for someone familiar with using the hostmonster.com interface.

This is the work I need done:
(the forms are already made)

form 1: I need the information in this form organized into an excel file that will show up in a file in my database, and that information must also be emailed to me. Must be able to support checkboxes, attachments, and radio buttons (an order form).

form 2: must simply be emailed to me ( a testimonial/comment form).

form 3: must be emailed to the person stated and must send an automatic personalized email (a refer-a-friend form).


This is an easy job, I'm just too lazy to do it myself. So the lowest bids will get the most attention. Please only bid if you know what you're doing.
