Description

This should be quick and easy for someone so I expect the actual bid to be representative of that as I am not prepared to pay more than a few dollars as it shouldn't take someone well versed in php and forms to figure this one out. I've made the form work and validated corerctly up to this point but having some difficulty in getting the checkboxes to email their details. Take a look at the attached file and whoever can come up with a working solution the quickest that works within the coding structure already on the form and at the lowest pm bid (below $30) gets the job.
