Description:	I have an existing highly customized cre loaded shopping cart installed on my server. You must be able to make these changes without breaking other parts of the site. Here's what I need done: Custom Form - create a mailto form with custom fields (select boxes, radio buttons, text fields, and 1 upload a file field) - when the form is submitted an email will be sent to a specific email address with the details of the request and an attachment Shipping Module - I need a shipping module to be installed and configured so that we can handle multiple vendors and shipping rates. It seems pretty simple, but I can't seem to figure out how this can be done. - the shipping looks like this domestic (50 states) usps ground - $x FedEx Saver - $x FedEx Premium - $x FedEx Saturday $x domestic (hawaii and alaska) FedEx Saver - $x FedEx Premium - $x FedEx Saturday $x international group 1 (select countries) fedex - $ dollars ups - $ dollars dhl - $dollars international group 2 (select countries) fedex - $ dollars ups - $ dollars dhl - $dollars The module needs to be flexible enough to create new vendors in the future and new zones. The shipping price should be calculated by price or weight or fixed fee. Small Bug Fixes - we installed QTpro, but for some reason the product attributes on the product page are not sorting by sort order - configure payment gateway (module already exists) - also need to bug fix usps auto rates module, I already have an account All work must be performed for a work for hire and an NDA must be signed. All code that has been changed must be commented with certain conventions so we can keep track of changes. These items must be done in a day or so - this is an urgent project.
