Description

Need PHP form that carries variables to the next page sends me the results and triggers a

Basically... a php form that not only forwards the persons info to us
but also carries the phone number (variable) into the next page... which use a redirect or something that would trigger a URL using a curl command in PHP.

I already have the curl command written.
