Hi All,

I need a MYSQL database and php to allow my users to add birth announcements.

I would like them to enter the following info when creating a new announcement

Baby Last Name
Baby First Name
Baby Middle Name (if applicable)
Hospital name
Mothers Name
Mothers Surname
Fathers Name
Date of Birth
Time of Birth
Baby Weight kg
Baby Weight LB (convert?)
Baby Length CM
Comment Text

Once entered the info needs to be approved by as moderator on my site.

This info needs to be displayed similar to how the birth notices are in the paper with all the available information, with options to sort on all fields. Then run reports such as all babies born on a certain day, all babies named jack etc etc

I would also like the option for the person entering the information to provide 5 email address for the info to be sent to (email a freind)

I am only really after the php and mysql as I can add this to my current website and use html

Please provide all relevant files and directorys plus a SQL to create the databse on my server.
