WHERE AM I

A simple one

I am in need of a PHP script to be called "WTFAI" that if I type in the IP it will return the following fields.

- IP typed in
- Country
- Region/State/province/county ( its usually one of those , we call it simple the REGION )
- Town/City ( i understand that some times this is not always possible , we will always call this the CITY )

The program must be totally self included, no access to an ourtside DB it may contain it own MYSQL tables V5 please and PHP no earlier then V4

Now this application must have the following as well when running

a ) A place you can type in the fields, and receive the information on the screen
b) The ability to send a sql query to the program to retrieve the information as CSV string
1) Given the IP return the Country
2) given the IP return the Country and Region
3) Given the IP return the Country and Region and City
c) I will assume you are getting the information needed from a sever some where that has this and the application
must have access to get this information for updating its tables , even if this is a once every so often request

d) produce a report ( text file )
Given Country , the following
Country / Regions it finds in alphabetical order

e) produce a report ( text file )

Given the Country and Region the following
Country / Region / City it finds in alphabetical order

Payment is only on 100% completion , if you need to buy a subscription to a service please include that as a separate item in your bid so we know the full on going costs but remember that this application will be on many servers of ours . Completion will be when you install the application on a server of our choice. We will not pay in advance or no code. payment by pay pal or Gaf only.
