Description

I need a RELIABLE!! php developer who can fix a bug in a existing script.

Task to do:
- Adding Add/Remove Button to form field (data filled into form field will be written into a database)
- Currently only one form (Title/Description) will be displayed but with the add button a new can be added if the user wants to add more entries
- To remove a form user can click on the remove button

I need to get this done today within the next hours, so if anyone is interested and available let me know.
