Description

I am creating a online form to insert into my MySQL db so that my staff can enter their timesheets. The user can select their name from a drop down list and then add in a day - click "add another" to add another day etc. The user could have multiple instances for the one say. Each entry must be assigned a job number which is selected from a table in the DB. See attached file for example (basic) layout. Need fast turnaround. I have a good basic with Php/MySql but dont have the time to look at this at the moment and need to get it done soon.

Also on the Week Beginning selection only "Mondays" should be selectable

When the user clicks "Add Another" I want the form to be repeated again underneath so that they can record as many days as they like in the one submission.
