Hi bidders,

I need a very simple vehicle management script. This will be integrated in another web-application, with graphics still under development, so I do not need a very detailed graphical details, but simply functionalities.

I attach 2 forms currently used by the company:
page #1: the log book for every vehicle (columns are self-explanatory)
page #2: a fund requisition form, shared by all vehicles

The company wants to automize data entry, instead of using Word/Excel files as they do now. Therefore, the script I want need to manage data exactly in the same format of attached forms.

== JOB TO BE DONE ==
I need a simple website, with the following pages:
1) log book (a list of records, with pagination; when you click on a row, it opens editing of the record, see step #2; also, a button to add a new record in log book)
2) editing of log book (add/edit a record, all fields of form in page #1 of attached PDF)
3) fund requisition (a list of records, with pagination; when you click on a row, it opens editing of the record, see step # 4; also, a button to add a new record in fund requisition)
4) editing of fund requisition (add/edit a record, all fields of form in page #2 of attached PDF)
5) admin panel: vehicles and drivers (list and edit details)
6) for every form, I need data validation

You DO NOT need to use some special graphical layout, because this would be useless (I will integrate in a large website). I will provide a simple HTML layout to the winner.

Forms DO NOT need to be printed. Only data entry and listing with pagination.

Data validation and Javascript: use ONLY jQuery, not other libraries

== DATA STRUCTURE ==
Database will be MySql. I'd need to be done using the following data structure:
- table VEHICLES (with unique ID, autoincrement)
- table DRIVERS (with unique ID, autoincrement)
- table LOGS (data for logbook form)
- table FUNDS (data for fund requisition form)

== OTHER NOTES ==
Code should be done in PHP4/5, Javascript can be used (only jQuery, no other libraries).

Code must be clear, well formatted and commented

This project is simple, involving only 4 listings and 4 forms. My budget is about 40/50 dollars.
