Description

I need a php form with about 30 fields and it will have a combination of fields and check boxes. here is what it looks like now.

Right now this is programmed using a mailto function. I want the data to be collected using php and then sent to my email. I want there to be a thank you and then redirect to the home page.

Simple job for someone who is good with php.
