I need a really simple form. I know how to do this but I don't have time.

This is a very simple PHP form that save the info in MYSQL database and send 2 emails

Form:
BUYER NAME:
BUYER LAST NAME:
ORDER #:
SELLER ID:
SELLER EMAIL:
TRANSACTION TYPE: SELECTION TYPE - ONLY ONE MUST BE SELECTED - "REFUND OR EXCHANGE"
COMMENTS: "TEXT WINDOW"

After submitting the form, all the information must show in a review page with a "confirm" and "edit" buttons

After "confirm' is submitted the info must be save in a mysql database

database name: database1
database user: user1
password: pass1

I have the table ready. Here is the info that should be in the table.

table name: returns

table fields:
id (automatic increments)
order_num
buyer_name
buyer_last_name
seller_ebay_id
seller_email
info (text window for comments)
refund (data 1 or 0)
exchange (data 1 or 0)

After info is submitted, the lat page should show a "Refund Number" from id field (automatic increments) and all the informations submitted with a message that say:

"Thanks for submitting your information. Please send this refund number to your buyer. The buyer must include this refund number when shipping the item back to us. This number will help us to accelerate the refund or exchange process."

The form will also sent the same information to SELLER EMAIL and to my REFUNDS EMAIL address

Use the information in this page to create the form, I will change the information to the correct php after is done, I know php already but I don't have time for writing this form.

This is a very simple form, I will select the best price.
