We have a simple php based sign up form. We need some very simple modifications made to the code.

This project is TIME SENSITIVE and needs to be complete by 2pm GMT! DO NOT BID if you cannot complete by 2pm GMT!

The budget is $50 as there are only a small number of issues to fix (6 issues in total).

You will have a total of around 5 hours to complete all issues!!
