I Have a super simple project.

I have a main page: http://www. bestmoversca .com/ (for the dates I want a calendar)

its the small form on the right, when someone completes it I want the data to move in to this form http://www.bestmoversca.com/estimate.html

Dont over bid I know how long it takes to do it, I just dont have the time.
