Description

SPECIFICATIONS:

Creation of a template PHP Form for searching data through Queries to a MySQL Database.

1. The MySQL database is already created
2. PHP Form templates is already created

I need the PHP mechanism for:

- POST data via PHP forms (using Multiple Lists, Checkboxes or Dropdown lists)
- Show MySQL results

I uploaded a sample Form Template.

I only need the code for 1 PHP template and i'll create the rest.
