Description

Hi I need an existing questionaire form reworked...it can be viewed here:

http://76.12.174.111/questionaire.php

The problems that need to be fixed are:

1. It should be in a popup window, like other forms on the site

2. User should not be able to advance to the next stage until the current stage is complete

3. Currently the submit button is separate from the "Next" / "Previous" buttons, this needs to change so that the Next Button changes to finish at the last stage

4. Their should be some sort of tabs/display at the top that informs the user which stage he is on and show which stages remain

5. I need a splash page that integrates with the existing site from which the user can click a link to open the popup window containing the questionaire..i will supply the contents for this page.

When a form is submitted the choices should be saved to an existing MySql database and also send to a designated email address.
