Description

I need two online forms that are printable. Users will fill in form fields with their info click submit and info will populate blank spaces in a pre-written story that I will provide. Then user can print story personalized with their info. Something like http://www.theanniversaryrose.com/love-letters.html

The stories are very short, about 350 words each and will print out on six or seven 8 ½ x 11 sheets with two 5 ½ x 8 ½ pages on each sheet, look at http://www.theanniversaryrose.com/example-pages.html for example of layout.

I will also want a few simple graphics in the printouts, I will supply graphics, you install them in forms.
