Description

I am wanting a simple script made that will do the following using PHP/MYsql.

A single input field for users to input a code, then be redirected to another page based on what code they typed in. If they input a code that has not been assigned a URL then they are sent to a 2nd URL that I have determined.

I would like a back end that I can type the codes and then the URLs associated to them. I would like it to be somewhat secure. I don't want people finding the codes unless given to them.

I would also like to have the ability to manage multiple input feilds on different pages. I attached an image of what I would like in the admin side.

Very simple not much more than that.
