Description:	I have an understanding of HTML code and HTML forms but I am having a hard time learning PHP. What I need to do is build a simple PHP registration form and have the results (a table with all the information from those who registered) available online (password protected) for the client to view. I want to use MySQL as the database. What I am looking for from you is: A: Build me the form in PHP with the HTML template that I will give you. B: Walk me through installing the form and setting up the MySQL database with my hosting (GoDaddy) Thank You.
