Hi,

I need the following done: I have 6 patient forms in html that need be added to my public website and need to send information to MySQL database. The data will need to be stored in a single table (tbl_patients). I also need that you create a small search page on my employee website (private, https side of my website) that will allow the employees to search for a person by their last name and (or) date of birth and then output the data as a pdf version of the above mentioned html form. Basically when let's say a person is found in the search results there will be a link (next to the found record) to download the pdf version of the *completed* html form that the patient has filled out. I belive that there is pdf creation class for php that you can use for this. This needs to be done for all 6 forms.

I only have a $100 budget for this project so do not bid higher than $100. The good news is that if you complete the listed task I will consider you for future similar jobs involving the same website.
