Description

Hello Bidders
I need a form amending which can be viewed on the PDF attached file.

This should take around 30 minutes for a good PHP programmer.

I have attached all the relevant files (I hope)for the old form.

Once completed then I will give you FTP access to my site for you to load up the new form then after testing (30 mins) I will make payment.
