http://www.shambhalacr.com/store/ was developed using oscommerce. We need to add several custom fields to the create an account page http://shambhalacr.com/store/create_account.php.

A field for Company Tax ID (Cedula Juridica) and Practicioner ID (# de titulo o licencia).

The above needs to post into the DB, and display within the backend Customers information area.
